Birth Certificates

Birth records on file in the City Clerk's office date back to 1728. Records 100 years old and older are public records. All records less than 100 years old are only available by request to the person named on the record or to a close relative.

Where do I obtain a copy of my Birth Certificate?

If you were born in Pawtucket, or if you were born in Rhode Island and your mother lived in Pawtucket at the time of birth, you may obtain your birth certificate in the City Clerk's Office:

or you may obtain your birth certificate at the Department of Health, Vital Records:

How to Obtain a Copy of your Birth Certificate:

  • Enter the City Clerk's Office, Room 301
  • Present a picture identification of yourself
  • Inform the clerk what size copy you want - wallet size or full size
  • State full name and date of birth for certificate, enclose photocopy of your photo ID.
  • State father's name and mother's maiden name
  • State wallet size of full size
  • Enclose check payable to the "City of Pawtucket"
  • If request is for certificate other than your own, follow the above steps and also state your relationship to that person.
  • Mail request to: City Clerk's Office, 137 Roosevelt Ave, Pawtucket, RI 02860

Cost:

$20.00 for the first certificate and $15.00 for each additional certificate when ordered simultaneously.

Wallet size birth certificates are available upon request. The fee is $20.00.
Wallet size birth certificates may be laminated for an additional $1.00.
WALLET SIZE BIRTH CERTIFICATES CANNOT BE USED FOR TRAVEL OUT OF COUNTRY!

 If you were born in another state and need a copy of your birth certificate, please go to the National Center for Health Statistics publication Where to Write for Vital Records (http://www.cdc.gov/nchs/howto/w2w/w2welcom.htm), an alphabetical directory of State and territory information, or inquire at the city or town clerk’s office.
